Who are the known members of the White Council, when was it formed, and who is the 'leader' of them?

I believe the only known members are Gandalf, Saruman, Galadriel, Elrond and Cirdan. There are also many other speculated members, such as Glorfindel, Celeborn and Radagst, but Tolkien never gave an actual list. The leader was originally Saruman the White, though Galadriel wanted it to be Gandalf the Grey. Later, when Saruman revealed himself to be "of many colours", Gandalf the White took over.

The etymology is uncertain... an interpretation could be that is real-world Gaelic (in which it would mean "mountain fortress")
Otherwise, 'Carn' could be derived from Sindarin, where 'caran' means red, and 'Dûm' is a Khuzdul (Dwarvish) word meaning 'halls' or 'mansions'
...a last option is that it is an unrecorded Mannish or Avarin language.
