Yes, the Legions of Isengard army list (from LoME) has Uruk-hai Shamans.

To my knowledge there are currently two different uruk shaman models.

Yup, they probably couldn't figure out anything else to stick in the new command set (though a horn blower would have worked so I'm still not quite sure why they brought out another). The first is waaay cooler IMO.
